Child and Family Connections (CFC)
Who's Who
Service Coordinator The service coordinator is the primary contact person for the family at a CFC. A service coordinator:
- Facilitates the evaluation / assessment process to determine eligibility
- Coordinates and monitors the Individualized Family Service Plan (IFSP)
- Supports the family throughout their enrollment in and transition from the Early Intervention System
- Informs the family of their rights and procedural safeguards within the Early Intervention System
Parent Liaison A parent liaison is the parent of a child who has received Early Intervention services. A parent liaison:
- Recognizes the unique concerns parents of children with special needs share
- Can offer families direct support
- Provides consultation and resources to Early Intervention team members
- Is credentialed as an Early Intervention provider through Provider Connections which is contracted through the Illinois Department of Human Services, Bureau of Early Intervention
Local Interagency Council The Local Interagency Council (LIC) is a forum for Early Intervention families, service providers, community members and CFC personnel to come together to:
- Share information
- Develop public awareness programs
- Coordinate developmental screenings and other Child Find activities
