Transition Information for Parents and Other Resources

Moving from program to program or within a program is called transition. In Early Intervention, transition could be moving:

• From hospital to home
• From Early Intervention to an early childhood program for three to five-year old children

Planning a smooth transition is very important for your child and your family.You are your child's best advocate!

• Illinois Student Records Keeper - For Parents of Students Who Receive Special Education Services
• Illinois PUNS Program (Prioritization of Urgency of Need for Services) a database of Illinois infants, children, adolescents, and adults with developmental disabilities who have needs for developmental disability services or supports. The PUNS database helps identify and plan for immediate needs.
